Treatment options for sleep apnoea include various forms of surgery and drugs but these are rarely used due to their cost, low succes rates and side effects. In March 2008, the N.I.C.E. stated that Positive Airway Pressure (PAP) therapy is the recommended treatment for moderate and severe obstructive sleep apnoea hypopnoea syndrome (OSA/HS) as well as for mild OSA, if weightloss and other measures failed.
Continuous Positive Airway Pressure (CPAP) has been the world-wide gold standard for OSA/HS therapy for many years and continues to be the option with the best clinical evidence and cost-effectiveness. The NHS has limited funds available as well as limited choice in equipment. Also, it is recognised that a number of patients fail to tolerate the CPAP machine and breathing mask. This CPAP compliance issue combined with innovation in recent years has seen an increased demand for Oral Appliance Therapy (OAT) for the treatment of OSA. Rarely funded by the NHS, OAT has to be funded privately in almost all cases.
